Unless otherwise specified, plan discounts do not apply to services performed by a dental specialist (i.e., periodontics, endodontics, orthodontics, and oral surgery) or dental hygiene products (e.g., chlorhexidine, toothbrushes, etc.).
Member savings and plan incentives are exclusive to participating dental offices. Exact member savings and availability of certain procedures may vary by location.
Services provided at no charge are limited to the quantity identified above during each annual contract term. Plan savings cannot be combined with any other offers or discounts.
Plan savings do not apply to the following procedures completed at our practice:
- Diagnostic treatment such as x-rays and exams (CDT Codes D0000-D0999)
- Preventive treatment such as cleanings and sealants (CDT Codes D1000-D1999)
- Restorative treatment such as fillings (CDT Codes D2000-D2999)
- Removable Prosthodontics treatment such as complete and partial dentures (CDT Codes D5000-D5999)
- Fixed Prosthodontics such as bridges (CDT Codes D6200-D6999)
- Adjunctive General Services such as anesthesia (CDT Codes (D9000-D9999)

This membership plan (the “Plan”) is NOT INSURANCE. Plan members pay periodic membership fees in exchange for access to discounts on certain identified health care services rendered by participating providers in accordance with the plan fee schedule. Plan members are obligated to pay providers directly for services rendered. Plan details, retail fees and member savings may vary by plan, provider and/or office location, please see plan terms and conditions for details.
Plan is an annual contract and includes an auto renewal feature. You can opt out of auto renewal at any time from the Member Portal.
Plan is administered by Clerri LLC, a discount medical plan operator. Clerri does not make payments to providers for services rendered to plan members. To obtain additional information about membership plans, please call (844) 965-5337 or email questions to member@clerri.com.
Arkansas, Florida, Louisiana, and Washington residents may be entitled to a full refund regardless of whether any discounted dental services have been provided to any member(s) during the current term. California, Florida, and Oklahoma residents who cancel outside the first 30 days may be entitled to a pro-rata refund. Please contact Member Support for details.
You may cancel your Plan membership at any time. You will receive a full refund of membership fees paid to Clerri if (i) the cancellation request is received within the first 30 days of the current annual contract term and (ii) no discounted dental services have been provided to any member(s) during the current term.
